Responsibilities:
- Provide a person-centred approach to all assessments and care plans.
- Contribute to collaborative working with other professionals.
- Ensure delivery of professional and clinical standards within the team.
Requirements:
- A Social Work Degree.
- 2 years post-qualifying experience.
- Social work registration with Social Work England.
