Job Opportunity: Social Worker & Social Worker Lead - Hospital Discharge and Assessment Team

Location: Midlands

Salary: £38p/hr for Social Worker | £40p/hr for Social Worker Lead

Accommodation: Up to £150 per week towards accommodation

We are seeking experienced Social Workers and Social Worker Leads to join our Hospital Discharge and Assessment Team in the Midlands. If you're passionate about making a real difference in people's lives and have a strong background in hospital or intermediate care, we want to hear from you!

Key Responsibilities:

* Undertake Mental Health Capacity Assessments with a focus on safeguarding and individual well-being.

* Work within a multidisciplinary team to provide holistic care for individuals in a hospital or crisis setting.

* Offer tailored support to individuals in the Loacal Authority, empowering them to regain control of their lives.

* Be adaptable and prepared to visit clients on-site, even on remote working days, when urgent care is needed.

Essential Requirements:

* Experience in Hospital or Intermediate Care settings.

* Knowledge of completing Mental Health Capacity Assessments and Safeguarding Concerns.

* Ability to work flexibly within a multidisciplinary team.

* Social Work qualification and registration with Social Work England.

* Completed ASYE year or equivalent demonstrable experience in a statutory social work setting.

* Strong knowledge of legislation, models, and theories relevant to Adult Social Work.

* Valid UK driving license and access to a car (exceptions may apply under the Equality Act).

Working Conditions:

* 2 days from home, 3 days in the office (with flexibility for urgent visits as required).

* First 2 weeks will be spent in the office for training and induction.
